Person-centered care
Support is planned around the person: their goals, strengths, preferences and pace.
Our approach
How we work matters as much as what we do. These principles guide our planning, our training and our day-to-day care.
Support is planned around the person: their goals, strengths, preferences and pace.
People make decisions about their own lives, and we help make those choices happen.
We teach, encourage and step back, so people can do more for themselves over time.
Every person is treated with their own voice, privacy and worth in mind.
We respect each person's culture, language, faith and family traditions.
We support people to take part in the same places and activities as everyone else.
We follow plans and practices that protect health and well-being.
Families and guardians are partners, kept informed and involved.
Our work connects to the goals in each person's plan, and we review progress with the team.
In practice
Before we plan anything, we want to know the person: what a good day looks like, what they want to learn, what worries them and who matters to them. Then we work with the family and support coordinator so everyone is pulling in the same direction.

Tell us what you or your family member needs. We will listen, explain the options and walk you through the next steps.
