For individuals & families
Getting started is simpler than you think.
Whether you are looking for support for yourself or someone you love, here is how it works with Rehoboth Broad, from first hello to the first day of services.
Get started
Six steps to support.
You do not need to have everything figured out before you reach out. We will help with each step.
Start step oneContact us
Reach out by form or message. Tell us who the support is for and how to reach you.
Tell us what services you need
Let us know which services you are interested in: habilitation and supported living, employment support or transportation.
We review availability and needs
We look at your needs and our availability, and talk with you about what support could look like.
We coordinate with your team
We work with you, your family and your support coordinator on the plan and authorization.
Complete enrollment and intake
We complete the enrollment and intake steps together, so we know the person and their goals.
Services begin once authorized
When the service is authorized, we schedule a start that works for the person.
Helpful to have ready
- The name of the person who would receive services
- The services you are interested in
- Your support coordinator's name, if you have one
- The best way and time to reach you
For families
You are part of the team. We will keep you informed, listen to what you see at home and work with you on goals. Questions are always welcome.
